Design Note: While this is categorized as a magical item, keep in mind that the soul grenade is not actually magical, but a soul item. Therefore, it follows the variant rule on spirit damage.

Wondrous Item, Very rare (requires attunement)

Created originally to show how powerful even a commoner's soul is, the soul grenade takes the spirit of a living host and turns it into destructive energy by ripping it apart and causing immense and rapid expansion that destroys the spiritual landscape around it, all at the cost of the user's own life. It is a tool limited within the ranks of the Anti-Magic League, and granted to their most kooky and overzealous crusaders to prove their dedication and the strength of their soul. Most members are not even aware of this particular item in the line of soul weapons.

Unlike a normal grenade, the soul grenade requires time to charge and absorb the soul of the attuned user. After a creature has spent at least 24 hours with the soul grenade attuned, it is considered armed and ready to use. As an action, the attuned user can throw the grenade up to 60 feet into an unoccupied space or detonate it where they stand. The effectiveness of the grenade depends on the soul of person using it, though even a weakling can create some chaos. For the radius of sphere of effect from the square of explosion, multiply the attuned creature's proficiency bonus by 5. For damage, add the attuned creature's Wisdom and Charisma scores together and multiply it by their proficiency bonus.

For example, a simple commoner has a proficiency bonus of 2 and Wisdom and Charisma scores of 10 each. Thus, if they use this grenade, they create a 10 foot radius sphere of effect, and all creatures in the sphere take 40 spirit damage.

Creatures in the area of effect cannot make a save to avoid this damage, but they can reduce it by an amount equal to their combined Wisdom and Charisma scores. Creatures that do not have souls, like certain constructs and undead, are immune to this damage.

After the explosion ends and the soul has been all but burned up, the attuned user dies immediately, as they have expended their spirit. They cannot be revived by anything short of a wish spell.
