You have a -1 penalty to str and con checks and saves if your skin is not exposed to water at least once an hour. After 2d6 days, you have a 50/50 chance of progressing to either stage 3a or stage 3b.

Stage 3a: Enlargement You grow to large size and you have a harder time breathing while dry. The penalty for having dry skin increases to a -2. After 2d4 days you progress to stage 4a.

Stage 4a: Poison Stinger You grow to huge size. You also grow a stinger that you can use for unarmed strikes, if you use it in this way, it deals 1 piercing damage and the target must make a DC 19 Constitution saving throw, taking 42 (12d6) poison damage on a failed save, or half as much damage on a successful one. It becomes harder to breathe, the penalty for having dry skin increases to a -4. After 1d6 days you become a purple worm, losing all your memories and becoming unaligned.

Treatment[edit]

<Wormification> can be cured with lesser restoration . After transforming completely, the only way to regain your original form is to die and be resurrected or through a wish spell or divine intervention.
