Surprise Attack. If the helicoprion surprises a creature and hits it with an attack during the first round of combat, the target takes an extra 7 (2d6) damage from the attack.

Water Breathing. The helicoprion can only breathe underwater.

Bite. Melee Weapon Attack: +5 to hit, reach 5 ft., one target. Hit: 12 (2d8 + 3) piercing damage. If the target is stunned, the bite deals an extra 3 (1d6) damage.

Tail. Melee Weapon Attack: +5 to hit, reach 10 ft., one creature. Hit: 10 (2d6 + 3) bludgeoning damage. If the target is Medium or smaller, it must succeed on a DC 12 Constitution saving throw or be stunned.


The helicoprion is a large, eel-bodied fish with a bizarre spiral of teeth in its lower jaw, resembling a circular saw. Despite its strange appearance, it is a swift and cunning predator, capable of striking from below with a lightning-fast tail whip followed by a vicious bite. Often mistaken for a shark at a distance, its movements are sleek, precise, and deadly.
It lurks in deeper coastal waters or sunken caverns, lying in wait to ambush prey that swims overhead. Fisherfolk speak of a sudden crack on the hull—and then silence, as swimmers vanish in red-tinged foam.
