System Shock (5e Variant Rule)

System Shock

This is a rule that tries to make combat more gritty and realistic. When a creature takes damage equal to 1/5th of it's max HP or more (rounded down) from a single attack or source, it must make a DC 15 Constitution saving throw or go into shock for one round. While in shock, a creature has disadvantage on attack rolls, saving throws, and ability checks, and any forms of speed it possesses are halved. Creatures which do not have a functioning metabolism or cannot feel pain, like undead or constructs, are immune to system shock. So are raging barbarians. This rule also adds a new feat which players can take:

High Threshold of Pain

Prerequisites: Constitution score of 13 or higher
You can shrug off far more pain than normal. Your hit point maximum increases by an amount equal to your level, and it increases by 1 every time you gain a level. You also are immune to the effects of system shock.
