This curse of Mutation can be applied to any item. The curse is activated when the item is held or worn. When this curse is activated roll 3 d6

1st Dice

1-2=Head

3-4=Upper Half

5-6=Lower Half

2nd Dice

Extra Features

Head

1-2=Eyes

3-4=Teeth

5-6=Ears

Upper Body

1-2=Fingers

3-4=Nipples

5-6=Ribs

Lower Body

1-2=Toes

3-4=Ankle bones

5-6=Leg Muscle

3rd Dice

Number of extra Features 1-6

For example 2 1 5 would give a character 5 extra eyes

This curse can be removed by a remove curse if the DM allows it otherwise it is permanent

Optional rules

For a more serious effect some mutations can be beneficial or bad

For example: A d100 can be rolled

Eyes 10 or lower the character goes blind 80 or higher the character gains a plus to spot checks equal to the number of eyes gained

Ears 10 or lower the character goes deaf 80 or higher the character gains a plus to listen checks equal to the number of ears gained

Leg Muscle 20 or lower the character loses 10 ft speed 90 or higher the character gains a plus 10 ft to movement

Moderate Transmutation; CL:10; Craft Wondrous Item, Baleful Polymorph; Cost: 60,000 gp.
