Back during the rise of the Empire of Netheril, an Archmage named Darmaseth rose to lichdom in a ritual that siphoned gargantuan amounts of negative energy into the ritual, making him a lich more powerful than even Orcus and Vecna. However, as everyone he knew either died of left him, he descended into madness. That, along with the massive amount of energy flowing through him, descended into the borderline realm of existence and non existence, becoming a vestige. Whenever someone contacts him, although only a handful know of his existence, they are momentarily spirited away to an alternate version of their realm, where Verminoth contacts them in his old lich form, which had looked like a pale human from preservation magic.
