Immortal Tormented cannot die; they are already dead! As the souls of sinners, they are doomed to eternal torment. If reduced to 0 HP, they return to full health after one round. If the damage used to kill them was Radiant damage, this duration increases to one hour.

Rest In Piece The spells greater restoration and wish can permanently kill a Tormented, allowing its soul to finally rest. The Tormented is always treated as a willing creature when either spell is cast in this way.


Slam. Melee Weapon Attack: +2 to hit, reach 5 ft., one target. Hit: 5 (1d6 + 2) bludgeoning damage.

The souls of those who've died and are set to any plane of Hell for eternal torment are granted by Hell a physical form. They are weak, and pathetic, and only exist to be tortured and tormented, to suffer pain and anguish. The release of death does not apply for them; their body will simply regenerate.

Tormented often appear in massive swarms of 10 or more, overwhelming potential foes in large numbers.
