To keep the game flowing, it might be better if it was: once per combat, upon taking damage make said will save or suffer ill effect. It could become rather annoying for the player as well as everyone else if you were making a will save every time you got hit.

Maybe not make is so they rage as immediate action, as the ability itself states that a barbarian can only rage on their turn, this would be an easy way to get around that rule, giving a barbarian a feat AND immediate rage (i think there is a feat that allows immediate raging which again gives him another advantage for taking this "flaw" but don't quote me on this part).
